Out-of-Network Provider (OON)

I am an out-of-network (OON) provider with all insurance companies. If you would like to submit a claim to your insurance, I can provide you with a monthly statement or "superbill" to submit to your insurance company for reimbursement. A clinical diagnosis is required in order to bill insurance.

Billing Statements

If you have any out-of-network coverage, I can provide an electronic billing statement or “superbill” which you will need to submit to your insurance. All services are to be paid in full at time of service, and then your insurance will reimburse you after you submit the claim.

Questions to Ask

Please contact your insurance company prior to meeting with me to determine your OON benefits. Here is a list of questions that you might want to ask them:

  • Do I have coverage for mental health counseling?

  • Have I met my deductible? If not, how much is left?

  • How many sessions will be covered per year?

  • How much is covered per counseling session with an OON provider?

  • Do I need a referral from my doctor?
